Before the massage session begins, it is important that a massage therapist understand the client’s medical health and what the client would like to accomplish from the massage, whether relief of achy muscles or simply to relax. A “Client Intake Sheet” is required for the first visit and is a Mandatory form per state compliance and regulatory. Subsequent visits will require a short form ONLY that gives an identification of the client’s areas of concern. The Licensed Massage Therapist will go over the form with you and will identify or clarify information from the intake sheet, where necessary. Prior to your massage, the therapist will explain the massage technique and ask if you have any questions. |
